Overview
Confidential Client is seeking a compassionate, entry-level Targeted Case Manager I to support patients with Substance Use Disorder (SUD) in Paris, KY. This role focuses on patient assessment, resource referral, care coordination, documentation, and professional compliance. Competitive pay: USD $40,000 - $45,000 per year. Full-time.
What You’ll Do
- Conduct patient assessments and determine eligibility for case management services.
- Develop individualized, integrated action plans aligned with patient needs.
- Refer patients to medical, social, educational, and employment resources and monitor follow-through.
- Provide crisis intervention and de-escalation support when needed.
- Coordinate reliable transportation solutions for patients to attend care appointments.
- Document case management activities and assessments accurately and promptly in electronic health records.
- Participate in supervision, treatment team meetings, quarterly in-service training, and TCM audits to ensure compliance and quality.
Required Knowledge, Skills & Abilities
- Excellent verbal and written communication skills.
- High empathy and a patient-centered approach to support recovery journeys.
- Able to work effectively with diverse patient populations.
- Strong problem-solving skills with a sense of urgency to address patient needs.
- Team-oriented and comfortable in multidisciplinary healthcare environments.
- Adaptable in a fast-changing clinical setting.
- Comfortable using multiple digital systems for documentation and communication; familiarity with MS Office (Outlook, Teams, Word) preferred.
- Commitment to compliance, integrity, and regulatory standards (HIPAA; familiarity with 42 CFR Part 2 desired).
- Prior experience with harm reduction is a plus.
Experience & Education
- Required: Minimum 1 year of experience working with adults in a human services setting.
- Required: Bachelor’s degree in a related Behavioral Science field.
- Preferred: Experience with harm reduction approaches and SUD populations.
Compensation & Benefits
- Base salary: USD $40,000 — $45,000 per year.
