Overview

Confidential Client (Social work) is hiring a Support Coordinator in Tucker, GA to support adults with Intellectual and Developmental Disabilities (I/DD). This full‑time role focuses on coordinating Medicaid waiver and community-based services, conducting home and community visits, developing person-centered plans, and working with families and providers to promote independence and community integration. The role carries a caseload of approximately 35–40 individuals.


Key Responsibilities

  • Maintain a caseload of ~35–40 individuals in the I/DD population
  • Conduct home and community-based visits to monitor services, safety, and satisfaction
  • Coordinate Medicaid waiver services and other community supports
  • Ensure services meet DBHDD/state standards, guidelines, policies and procedures
  • Develop and communicate person-centered treatment plans with participants, families, and providers
  • Attend meetings with families, providers, and regional staff as requested
  • Complete timely, accurate documentation (weekly and as required)
  • Manage time independently to meet productivity benchmarks and deadlines

Required Skills & Experience

  • Bachelor’s degree in social sciences or equivalent
  • Minimum 1 year experience working with I/DD individuals
  • Proficiency with Microsoft Word, Excel, PowerPoint, and Outlook
  • Experience in program development for individuals with developmental disabilities or behavioral health needs
  • Strong relationship-building skills with clients, families, providers, and community stakeholders
  • Person-centered planning and advocacy experience
  • Excellent time management and organizational skills; detail-oriented for documentation
  • Ability to maintain confidentiality of sensitive information
  • Reliable access to a laptop and internet connection
  • Comfortable with regular in‑community travel within the assigned region

Compensation & Schedule

  • Employment type: Full time
  • Estimated pay: $64,590 per year (estimate; actual compensation may vary based on experience and location)
  • Typical schedule: daytime hours with travel for home/community visits and occasional evening or regional meetings as needed
