What You’ll Do
We are hiring a Spanish-speaking Home Health Physical Therapist Assistant (PTA) to deliver skilled, patient-centered care in home and facility settings. This role supports post-surgical patients, older adults, and people with neurological conditions through scheduled home visits, route-based visits, and occasional facility work.
- Assess patients and develop individualized care plans
- Conduct home and facility-based patient visits
- Provide therapeutic exercises, mobility and gait training
- Perform manual therapy and gait analysis as appropriate
- Educate patients and caregivers; coordinate care with other providers
- Document clinical services accurately in electronic health records (EHRs)
Minimum Qualifications
- Active state physical therapy or assistant license (PT or PTA) in good standing
- Current CPR certification
- Spanish language proficiency (verbal) — able to communicate clinically with Spanish-speaking patients
Preferred Qualifications
- Entry-level to early-career experience in physical therapy or home health settings
- Bachelor’s or Master’s degree in Physical Therapy or related field (if PTA role, PTA credential required)
- Familiarity with EHRs and clinical documentation best practices
- Strong skills in patient education, care coordination, and clinical communication
