About the Role

Confidential Client is hiring a compassionate Social Worker in Hico, TX. This full-time, entry-level friendly role supports patients and families by assessing social and emotional needs, developing individualized care plans, coordinating community resources, and working with an interdisciplinary healthcare team to deliver patient-centered care.

What You’ll Do

• Conduct initial and ongoing assessments of patients’ social, emotional, and psychosocial needs
• Develop and implement individualized care plans and treatment goals
• Provide counseling, education, and supportive interventions for patients and families
• Coordinate referrals and community resources (housing, transportation, benefits, counseling, etc.)
• Collaborate with nurses, physicians, and other care team members to support discharge planning and continuity of care
• Maintain accurate, timely documentation of assessments, plans, and patient interactions

Requirements

• Bachelor’s degree in Social Work or a related field, or equivalent experience
• 0 years experience — entry-level candidates and recent graduates are encouraged to apply
• Strong communication, organization, and interpersonal skills
• Ability to work collaboratively on an interdisciplinary healthcare team
• Must be authorized to work in the United States
