What You’ll Do

Careertakes is supporting a Confidential Client seeking a Social Worker / Discharge Planner to perform clinical and discharge-planning work in a healthcare setting. The role focuses on assessing patients’ medical and psychosocial needs, coordinating safe transitions of care, and communicating with families, clinicians and external resources.

  • Conduct comprehensive medical and psychosocial assessments to develop individualized discharge plans
  • Coordinate with multidisciplinary teams (nursing, physicians, rehab, case management) to identify safe discharge placements
  • Communicate with patients, families and external agencies to discuss discharge options and available resources
  • Document assessments, plans and communications accurately in the patient medical record / EHR
  • Provide emotional support, coping strategies and brief counseling to patients and families
  • Identify and discuss financial/insurance alternatives and community support services with patients and families
  • Lead or participate in interdisciplinary discharge planning meetings and care rounds
  • Serve as a liaison between Confidential Client and external referral sources
  • Present and attend in‑services to maintain and enhance professional knowledge

Qualifications

  • Bachelor’s degree in Social Work or a related field required
  • Entry-level candidates considered; clinical experience in healthcare preferred
  • Licensure: eligible to practice as a Social Worker in the State of Wisconsin, or ability to obtain Wisconsin licensure within 12 months of hire
  • Must be able to read, write, speak and comprehend English for clinical documentation and patient communication

Skills & Abilities

  • Strong assessment, clinical judgment and care-planning skills
  • Effective verbal and written communication with multidisciplinary teams and external agencies
  • Proficiency with clinical documentation and comfort using electronic health records (EHR) systems
  • Knowledge of community resources, discharge placement options and financial assistance programs
  • Empathy and crisis-intervention skills; ability to support patients and families during transitions

Compensation & Benefits

  • Estimated base salary: $73,185 per year (estimated)
  • Benefits may include health insurance, retirement savings and paid time off; exact package is provided by Confidential Client
