What You’ll Do
Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders. You will help clients live as independently as possible in their chosen community by:
- Conducting outreach and engagement with clients and families
- Helping develop and implement individualized care plans
- Linking clients to community resources, benefits, housing, and medical/behavioral services
- Coordinating appointments and providing or arranging transportation when needed
- Maintaining accurate progress notes and documentation in electronic records
- Participating in team meetings, supervision and trainings as required
- Supporting crisis de-escalation and safety planning as needed
Shifts & Schedule
Short Hour | AM
Shifts: 8:00 AM - 4:30 PM
Days: Friday - Saturday
Compensation
Expected starting wage range: $23.00 - $23.71 per hour. Our client applies geographic differentials to pay ranges; the final pay for this role will be based on the location where the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
Minimum Qualifications (Must Have)
- High School diploma or G.E.D. equivalent and two (2) years of direct services in mental health or community services; OR
- Associate’s degree and one (1) year of related experience; OR
- Bachelor’s degree in Social Services or a related field
- Valid, current driver’s license and personal vehicle insurance with your name listed as a driver
- Willingness to use your personal vehicle to transport clients to appointments and groups when required
- Willingness to use company vehicles when available and directed
Preferred Experience & Skills
- Experience working with people with serious mental illness and co-occurring disorders
- Strong documentation and time-management skills
- Ability to work effectively in a trauma-informed, culturally humble setting
- Comfortable working with youth, families, and community partners

The Kern Youth Family Resource Center is a family-friendly, trauma-informed program that provides youth and families with timely support tailored to their needs. As part of the local continuum of care, the program partners with families and community providers to help youth navigate challenges and connect with ongoing supports.
