The Human Services division runs outcome-focused employment programs that connect participants to training, job readiness, placement, and supportive services.

Position summary

The Service Coordinator provides strength-based, holistic case management for participants in the EARN & Work Ready programs. This role is the main point of contact for participants, conducting assessments, creating measurable plans, coordinating services across internal teams and community partners, and connecting participants and families to resources.

What You’ll Do (Responsibilities)

• Conduct comprehensive assessments covering supports, child care, transportation, housing, education, criminal history, employment, medical/behavioral health, substance use, barriers, strengths, and goals.
• Administer and interpret approved assessment tools and ensure program policy compliance.
• Work collaboratively with participants to create measurable life and work goals and hold Monthly Planning Meetings to review Life Goal Plans.
• Develop Program Service Plans with participants, internal teams, and outside providers; meet program timelines and document progress.
• Coordinate services across internal staff, community providers, and funders; maintain up-to-date knowledge of community resources.
• Advocate for participants and families to ensure their priorities shape service delivery.
• Maintain accurate digital case files and case notes following program and funder guidelines.
• Follow applicable regulatory guidance and communicate production/job expectations with site management when relevant.

Qualifications

• Education & experience (one of the following):
  • High school diploma or equivalent AND 4+ years relevant experience, OR
  • Associate’s degree in Psychology, Rehabilitation Science, Social Work, or related field AND 2+ years relevant experience, OR
  • Bachelor’s degree in Psychology, Rehabilitation Science, Social Work, or related field AND 1+ year relevant experience.
• Experience locating and coordinating resources for at-risk or vulnerable populations. Relevant experience may include goal planning and supporting clients with poverty-related needs, child care, transportation, food and housing insecurity, unemployment/underemployment, criminal background challenges, mental health and substance use issues, and ID/DD.

Required Clearances & Certifications

• Candidates must complete background checks assigned by Confidential Client, including a National Criminal Background check, prior to their first day of employment.
• Candidates are expected to provide current, valid clearances prior to their first day of employment (Child Abuse Clearance, FBI fingerprints clearance, and PATCH or equivalent).

Hours & Compensation

• Status: Regular, Full-time, Non-exempt.
• Schedule: Monday–Friday, 8:00 AM – 4:00 PM.
• External hiring range: $19.18 - $21.16 per hour.
• Travel: Occasional travel may be required.
