Position Overview
The RN Case Manager I (RN CM I) will coordinate discharge planning and post-acute care for patients, working closely with patients, families and the interdisciplinary care team. This is a temporary, entry-level RN case management role that requires progression to RN CM II within one year. The role is based in Ojai, CA and is full-time.
What You’ll Do
- Collaborate with nurses, physicians, social workers and other care team members to develop and execute safe, timely discharge plans
- Coordinate post-acute care services and referrals (home health, SNF, outpatient resources)
- Collect and document clinical information to support admission criteria, continued-stay review and appropriate level-of-care decisions
- Use utilization review tools (e.g., InterQual) and EHR systems (Allscripts, Meditech, Epic) as required
- Communicate with patients and families about discharge plans and available resources in a compassionate, culturally sensitive manner
- Maintain accurate case notes, complete required documentation and support quality/compliance metrics
Required Qualifications
- Associate Degree in Nursing (ADN)
- Current California Registered Nurse (RN) license
- Current Basic Life Support (BLS) certification
- Ability to work full time in Ojai, CA and to perform duties listed
- Authorization to work in the U.S.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Bilingual in English and Spanish
- Previous experience with InterQual
- Experience with Allscripts, Meditech, or Epic
- Prior case management or utilization review experience
Compensation & Employment Type
- Pay range: $54.08 - $81.59 per hour (hourly)
- Employment type: Full time
