Role overview

Confidential Client is seeking a Part‑Time RN Acute Transitional Care Manager to coordinate safe, timely transitions from acute or skilled nursing settings back to home or alternate care for members with complex medical needs. This role focuses on assessment, discharge planning, vendor coordination, and close follow‑up to reduce readmissions and ensure continuity of care.


What You’ll Do

  • Conduct timely telephonic and clinical assessments of identified members (physical, psychosocial, environmental, and functional needs).
  • Develop individualized, mutually agreed plans of care with measurable goals and target dates.
  • Coordinate with hospitalists, inpatient case managers, social workers, telephonic care coordinators and outpatient providers to create and implement safe discharge plans.
  • Attend patient care conferences/rounds and act as the primary coordinator for transitional care activities.
  • Arrange and confirm post‑acute services, follow‑up medical appointments and vendor services.
  • Perform telephonic outreach within 48 hours post discharge and reassess member status.
  • Document all case management interactions and interventions according to departmental guidelines.
  • Monitor utilization trends and refer cases for risk management, peer review or quality issues as appropriate.
  • Participate in on‑call rotations for after‑hours and weekend referrals when needed.
  • Coach and support peers and other staff on best practices in transitional case management.
  • Maintain confidentiality of protected health information (PHI) and adhere to applicable laws, accreditations, and client policies.

Required qualifications

  • Minimum three (3) years acute hospital discharge planning or case management experience as a Registered Nurse (RN).
  • Active Registered Professional Nurse license in Georgia OR Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in Georgia.
  • High school diploma or GED required; nursing degree preferred.
  • Strong telephonic assessment, clinical reasoning and care coordination skills.
  • Functional computer skills and proficiency with documentation systems.
  • Ability to travel within the Atlanta metro area (≈20% travel).

Preferred

  • Bachelor of Science in Nursing (BSN) or four (4) years of directly related experience.
  • Prior complex case management experience and familiarity with Medicare populations.
  • Experience working onsite at skilled nursing facilities (SNF) and coordinating vendor services.

Hours, location & compensation

  • Job Type: Part‑time (Standard) — Scheduled weekly hours: 24 (Day shift)
  • Workdays: Varied (Sun–Sat potential)
  • Onsite requirement: Onsite at client‑designated locations and SNFs in the Atlanta, GA area
  • Travel: Up to 20% within the Atlanta metro area
  • Pay Range: $47.37 – $60.85 per hour (posted by Confidential Client)

Note: The posted pay range reflects possible base pay for this role and does not include total rewards. Final base pay will be determined by market data, internal alignment, and candidate experience, education, certifications and location.
