Overview

Confidential Client is seeking a Rehabilitation Registered Nurse (Rehab RN) to join their care team in Waupaca, WI. This full-time clinical role focuses on patient-focused rehabilitation care, interdisciplinary collaboration, and improving patient outcomes through individualized care planning.

Responsibilities

• Conduct comprehensive patient assessments to determine rehabilitation needs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Administer medications and treatments as prescribed
• Monitor patient progress and update care plans to meet rehabilitation goals
• Provide patient and family education on treatment plans and self-care techniques
• Coordinate with physical, occupational, and speech therapists to optimize outcomes
• Accurately document patient care in the electronic medical record (EMR)
• Ensure compliance with safety, infection control, and clinical protocols
• Assist in discharge planning and transitions of care
• Maintain professional knowledge of best practices in rehabilitation nursing

Qualifications

• Current and valid Registered Nurse (RN) license for practice in Wisconsin
• BSN preferred
• Minimum ~2 years’ experience in rehabilitation, long-term care, or related nursing setting preferred (entry-level candidates with relevant clinical experience will be considered)
• Certification in Rehabilitation Nursing (CRRN) preferred or willingness to obtain
• Basic Life Support (BLS) required; ACLS preferred
• Proficiency with EMR systems
• Strong clinical judgment, communication, and interdisciplinary teamwork skills
• Patient-centered approach with excellent critical-thinking abilities
