About the Role
Join Confidential Client as a Rehabilitation Registered Nurse (Rehab RN) in Waupaca, WI. In this part-time role you will deliver patient-centered rehabilitation nursing care, collaborate with therapists and physicians, and support patients to regain function and independence. What You’ll Do
- Conduct comprehensive patient assessments to identify rehabilitation needs and goals
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Administer medications and treatments as prescribed
- Monitor patient progress and update care plans to meet rehabilitation objectives
- Educate patients and families on treatment plans, self-care, and safe discharge practices
- Collaborate with physical therapists, occupational therapists, speech therapists, and other rehabilitation professionals
- Accurately document patient care and maintain up-to-date medical records in the EMR
- Ensure compliance with health, safety, and infection-control policies
- Assist with discharge planning to ensure continuity of care after rehabilitation
- Stay current on evidence-based practices in rehabilitation nursing
Qualifications
- Valid and current Registered Nurse (RN) license; applicants must hold (or be eligible for) an active RN license to practice in Wisconsin
- Bachelor of Science in Nursing (BSN) preferred
- Minimum of 2 years of nursing experience in a rehabilitation, long-term care, or related clinical setting preferred; entry-level candidates with strong relevant clinical experience may be considered
- Certification in Rehabilitation Nursing (CRRN) preferred or willingness to pursue
- Basic Life Support (BLS) certification required
- Advanced Cardiovascular Life Support (ACLS) certification preferred
- Proficiency with electronic medical records (EMR) systems
- Strong communication, critical thinking, and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Authorization to work in the United States
