Position Overview
Confidential Client is seeking a Registered Nurse (RN) to join an outpatient PACT (Patient Aligned Care Team) setting in Tomah, WI. This full-time contractor role supports high-quality, patient-centered outpatient care. The position requires active nursing licensure, clinical competency in outpatient settings, and the ability to deliver compassionate care to a diverse patient population.
Key Responsibilities
- Provide direct outpatient nursing care consistent with professional standards and Confidential Client policies
- Triage, assess, plan, implement, and evaluate patient care for assigned caseloads
- Administer medications, perform treatments, and document interventions accurately
- Collaborate with interdisciplinary team members (physicians, social work, pharmacy, etc.) to coordinate care
- Participate in patient education, preventive care, and chronic disease management programs
- Support continuous quality improvement, safety practices, and evidence-based care initiatives
- Maintain accurate, timely patient records and comply with HIPAA and facility documentation standards
Minimum Qualifications (Required)
- Current, full, active, and unrestricted registration as a professional nurse (RN) in a U.S. State, Territory, Commonwealth, or the District of Columbia. Candidates must obtain Wisconsin RN licensure prior to or upon hire if required by state regulations.
- Graduate of an accredited school of professional nursing (ACEN or CCNE) — or equivalent degree validated by an approved equivalency evaluation for graduates of foreign programs
- English language proficiency sufficient for safe, effective patient care and documentation
- Ability to meet the physical demands of the role (standing, walking, lifting, safe patient handling)
Notes on exceptions and pathways:
- Graduate Nurse Technician (GNT) Exception: candidates meeting education requirements but not yet licensed may be appointed temporarily while pursuing licensure (per Confidential Client policies). Extensions may be considered on a case-by-case basis.
- Grade and level placement will be determined by education and clinical experience according to the applicable qualification framework.
Preferred Qualifications
- Minimum 1 year of professional RN experience
- 2 years of outpatient RN experience strongly preferred
- Experience with ambulatory care workflows, chronic disease management, and patient education
- Demonstrated ability to work collaboratively in team-based primary care settings
Compensation & Employment Details
- Salary: $77,771 - $147,657 per year (range provided by Confidential Client)
- Employment type: Contract, Full Time
- Location: Tomah, WI (on-site outpatient position)

Physical & Work Environment Requirements
- Role includes periods of standing, walking, sitting, moderate lifting, repeated bending, or kneeling
