What You’ll Do

This Registered Nurse (Patient Aligned Care Team — PACT) will deliver primary and preventive care as part of an interdisciplinary PACT team. Responsibilities include:

• Provide nursing assessment, triage, and direct patient care for assigned panel members.
• Coordinate care and referrals with primary care providers, specialists, and ancillary services.
• Educate patients and families on diagnoses, medications, self-care and preventive health.
• Document assessments, care plans, and patient encounters in the electronic health record accurately and on time.
• Participate in quality improvement initiatives and evidence-based practice activities.
• Support population health tasks such as panel management, chronic disease follow-up, and medication reconciliation.
• Follow safe patient handling procedures and use mechanical lifts when required.
• Communicate effectively with veterans/patients, caregivers, and team members to optimize outcomes.

Qualifications

Minimum and program-specific qualifications (select the level that applies):

• English language proficiency required (written and spoken).
• Graduate of an accredited professional nursing program (ACEN or CCNE) OR equivalent evaluated foreign education with required validation.
• Current, full, active, and unrestricted RN registration/license in any U.S. state, territory, commonwealth, or the District of Columbia.
• Graduate Nurse Technician (GNT) Exception: candidates meeting education requirements but still pursuing licensure may be appointed temporarily under the GNT policy.
• Grade/level criteria (summary):
  • Nurse I, Level I — Associate Degree (ADN) or Diploma in Nursing.
  • Nurse I, Level II — ADN/Diploma + 1 year specialized nursing experience OR BSN.
  • Nurse I, Level III — ADN/Diploma + 2 years professional nursing experience (one year equivalent to Level II) OR BSN + 1 year professional experience OR MSN/no experience.
  • Nurse II — BSN + 2 years professional nursing equivalent to Nurse I, Level III OR MSN + 1 year specialized experience.
  • Nurse III — MSN + 2 years specialized nursing experience (one year equivalent to Nurse II) OR Doctoral-level qualifications as described in the qualification standards.

Preferred / Nice-to-Haves

• BSN degree or higher.
• Experience in primary care, care coordination, or population health.
• Familiarity with electronic health records and quality improvement methodologies.
• Demonstrated leadership, patient education, and care-management skills.

Physical & Other Requirements

• Role may include walking, bending, stooping, transferring patients/objects, and some lifting. Use of mechanical lifts provided when appropriate.
• Extended periods of sitting or standing may occur.

• Salary range (provided): $89,703 - $166,144 per year
• Location: Gilbert, AZ
