What You’ll Do

Confidential Client is hiring a Registered Nurse (RN) for Home-Based Primary Care. This remote-friendly position supports veterans and other patients in their homes and virtually, delivering comprehensive, patient-centered nursing care.

• Perform comprehensive nursing assessments in home and virtual settings
• Develop, implement, and update individualized care plans
• Provide chronic disease management, medication reconciliation, and patient education
• Coordinate care with interdisciplinary teams, social work, and community resources
• Conduct telehealth visits and document encounters thoroughly in the electronic health record
• Support care transitions, referrals, and case management activities
• Maintain professional nursing standards, infection control, and safety during home visits
• Meet physical requirements of the role (extended walking/standing, moderate lifting, bending/kneeling)

Qualifications (Required)

Candidates must meet applicable federal and state requirements for professional nursing roles.

• Current, full, active, and unrestricted RN license in a U.S. state, territory, commonwealth, or the District of Columbia
• Graduate of an accredited professional nursing program (ACEN or CCNE), or equivalent validated foreign nursing education plus appropriate licensure
• English language proficiency sufficient for safe, effective written and spoken patient care
• Ability to meet the physical demands of home-based care (walking, standing, lifting, bending)
• Willingness to complete any employer-required onboarding or credentialing

Notes on education/grade equivalencies (used for internal grade placement):

• ADN/Diploma and relevant experience or BSN/MSN/Doctoral degrees will be considered per the client’s qualification standards and grade-level definitions
• Candidates actively enrolled in bridge programs may be eligible for temporary appointment while pursuing licensure (per client policy)

• Prior experience in home-based primary care, home health, or case management
• Experience with telehealth delivery and remote patient monitoring
• Strong clinical judgment and experience coordinating care across settings
• Familiarity with evidence-based practice, quality improvement, and patient-centered care models

Compensation & Employment Type

• Salary range: $75,487 – $153,842 per year (provided by source)
• Employment type: Full time / Contractor (depending on assignment and client policies)

• Primary location: Jefferson Barracks, Missouri
• Work model: Remote-friendly / Telecommute with required in-person home visits as assigned
• Applicant location requirement: Applicants must be eligible to work in the United States
