Overview

Confidential Client is seeking an experienced Registered Nurse (RN) to join a Patient Aligned Care Team as a Care Manager in an outpatient setting. This role supports coordinated, patient-centered care for veterans and other patients, emphasizing clinical assessment, care planning, care coordination, and patient education.


What You’ll Do

  • Provide outpatient nursing care as part of a multidisciplinary Patient Aligned Care Team (PACT).
  • Serve as a care manager for assigned patients, coordinating care across providers, services, and community resources.
  • Conduct clinical assessments, develop and update individualized care plans, and monitor outcomes.
  • Educate patients and caregivers on disease management, medications, and follow-up care.
  • Communicate regularly with primary care providers, specialty care teams, and social services to support continuity of care.
  • Use electronic health records (EHR) and other clinical systems to document care, order and track tests, and manage referrals.

Required Qualifications

  • Current, full, active, and unrestricted RN license in any U.S. State, Territory, or the District of Columbia.
  • Graduate of an accredited school of professional nursing (ADN, Diploma, BSN, or higher) — accreditation by ACEN or CCNE preferred.
  • English proficiency in written and spoken communication.
  • Demonstrated clinical nursing experience appropriate to the grade/level being considered (see scope below).
  • Ability to work in an outpatient setting and coordinate care across teams.
  • Must meet any temporary appointment or licensure conditions (e.g., Graduate Nurse Technician exceptions) as permitted by state and federal regulations.

Grade/level guidance (employer will assess and assign grade/level during hiring):

  • Nurse I (Levels I–III): ADN/diploma or BSN with varying required experience.
  • Nurse II: BSN plus 2 years of experience (or MSN equivalencies).
  • Nurse III: MSN/Doctoral degree plus required specialized experience.

Preferred Qualifications

  • Prior experience as an RN care manager or in a Patient Aligned Care Team.
  • Experience working with veterans, primary care populations, or complex chronic disease management.
  • BSN or higher and evidence of continuing professional development.
  • Experience with EHR systems and care coordination tools.

Compensation & Work Type

  • Salary range: $85,610 – $173,974 per year (range provided by the hiring organization).
  • Employment type: Contractor, Full-time.
  • Location: San Antonio, TX (on-site / outpatient clinic).
