What You’ll Do

As a Psychiatric Rehabilitation Specialist (Confidential Client), you will support people with behavioral health needs by helping them build skills, stay engaged in treatment, and connect to community resources.

• Support patients through outreach calls, telehealth, and in-person visits to identify barriers and implement new skills
• Coach patients to meet treatment and wellness goals and improve engagement
• Reinforce the importance of following up with healthcare providers and support medication adherence (coordination with pharmacies, medication packs, pill organizers, home delivery)
• Conduct screenings and assessments in the Electronic Health Record (EHR) to measure progress and needs
• Provide education and consultative support to multidisciplinary behavioral health teams (prescribers, clinicians, care coordinators)
• Present at case conferences to raise awareness of Psychiatric Rehabilitation services
• Connect patients to community resources and coordinate care with external providers
• Travel locally for one-on-one sessions or virtual appointments as needed
• Retrieve and enter clinical and non-clinical data from internal EHRs and regional health systems to support patient goals
• Participate in program projects and service development for Psychiatric Rehabilitation
• Interact respectfully and supportively with individuals and families from diverse cultural and socioeconomic backgrounds
• Learn and follow Confidential Client policies and adhere to privacy/confidentiality regulations

Qualifications

You meet the minimum requirements with one of the following education/experience combinations:

• Bachelor’s degree plus 6 months relevant, patient-facing behavioral health experience — required

• OR -

• Associate’s degree plus 1 year relevant, patient-facing behavioral health experience — required

• OR -

• High school diploma or equivalent plus 2 years relevant, patient-facing behavioral health experience — required

Additional requirements and preferences:

• Individuals with lived experience are preferred
• Valid New York State driver’s license and reliable transportation for regular local travel
• Experience using Electronic Health Record (EHR) systems preferred; willingness to learn new software
• Familiarity with reporting and entry-level data analytics preferred
• Strong communication, coaching, and culturally responsive interpersonal skills

Location & Schedule

• Clinic location: 637 Davison Rd, Lockport, NY — frequent local travel required
• Schedule: Monday–Friday day shifts with occasional evening flexibility as needed

Compensation & Benefits

• Pay range: $20.00–$24.00 per hour. Starting pay depends on experience and education; compensation reviewed annually.
• Sign-on bonus: Eligible for up to $1,500 for commitment to work at designated Certified Behavioral Health Clinics for at least one year (terms apply; changes in hours or location may affect eligibility)
