Position summary
Confidential Client is seeking a Psychiatric Rehabilitation Specialist (CCBHC) to join a community-focused behavioral health team in Buffalo, NY. This full-time role supports people with behavioral health needs through outreach, skill-building, care coordination, and connection to education and employment resources.
What You’ll Do
You will provide direct rehabilitation services that help individuals gain skills, access services, and strengthen supports for recovery and community integration.
- Provide ongoing rehabilitation support via outreach calls, telehealth, and face‑to‑face contacts.
- Help individuals identify barriers to recovery and develop practical skills for independence.
- Support treatment, wellness, and recovery goals through skill-building interventions and engagement strategies.
- Serve on the Open Access team to perform initial needs assessments and ensure timely connection to clinical and community supports.
- Assist with educational goal-setting and referrals (training programs, GED, college partnerships).
- Support vocational and employment goals: career planning, resume help, interview prep, job‑readiness coaching, and referrals to workforce programs.
- Coordinate community linkages and monitor progress; follow up with individuals and service providers.
- Participate in multidisciplinary case conferences and treatment team meetings to develop person-centered plans.

Location & Schedule
- Primary location: Buffalo, NY (clinic at 3020 Bailey Ave). Frequent local travel required.
- Typical hours: Monday–Friday daytime shifts; occasional evening flexibility as needed.
Compensation
- Hourly range: $20.00–$24.00 (starting pay depends on experience and education; adjustments evaluated annually).

Minimum Qualifications
Applicants must meet one of the following education/experience combinations:
- Bachelor’s degree AND 6 months of relevant, patient-facing behavioral health experience; OR
- Associate’s degree AND 1 year of relevant, patient-facing behavioral health experience; OR
- High school diploma or equivalent AND 2 years of relevant, patient-facing behavioral health experience.
Additional requirements/preferred skills:
- Individuals with lived experience are preferred.
- Valid NYS driver’s license and reliable local transportation required.
- Experience with Electronic Health Records (EHR) and willingness to learn new software preferred.
- Familiarity with reporting and basic data analytics preferred.
Physical & Other Requirements
- Frequent standing/sitting alternation; mobility between nearby worksites.
- Repetitive wrist/hand/finger movements.
- Ability to handle light duties and move items up to 20 lbs (occasionally up to 50 lbs).
- This position involves in-person services and engagement with community partners.
