Primary Care RN Case Manager (Open Rank: Entry - Senior)
The Primary Care RN Case Manager at Confidential Client provides comprehensive care coordination and case management for patients with complex needs across multiple service lines. This role involves clinical nursing duties, health coaching, quality improvement initiatives, and training/support of students and medical assistants, with increasing independence and leadership responsibilities at higher ranks. The position is onsite at Sheridan Health Services, focusing on integrated, patient-centered care within a diverse community health setting.

What You’ll Do
This is an outpatient primary care case management role focused on care coordination, clinical triage, and population health for patients served by a nurse-managed community health clinic. Responsibilities include:
- Care coordination and case management across service lines (adult and pediatric primary care, maternal/child, behavioral health, psychiatry) — coordinating referrals, specialty access, and community resources.
- Manage critical and complex test results, communicate results to patients, triage and assess urgency, provide health education, and develop follow-up plans in collaboration with primary care providers.
- Conduct independent and collaborative nursing visits for chronic disease follow-up (blood pressure, diabetes), medication-assisted treatment check-ins, medication teaching (including self-injection instruction), health coaching and nutrition counseling.
- Assess social determinants of health, identify barriers to care, and create individualized care plans that address access needs.
- Perform telephone triage and collaborate with providers to resolve patient care needs between visits.
- Participate in population health and quality improvement activities to improve preventive care access, immunization rates, and chronic disease measures. Track and document progress in the electronic medical record.
- Engage in community outreach and patient education events to support clinic goals.
- Provide student precepting, orientation, and training of medical assistants and support staff; may supervise medical assistants, patient navigators, or referral coordinators as appropriate.
- Support grant-related activities and metrics as required by clinic programs.
Note on rank levels
- Entry level: duties performed with guidance and in a training capacity.
- Intermediate level: duties performed with supervision and limited independence; requires 1 year RN experience.
- Senior level: duties performed fully independently; requires 2 years RN experience.
- Principal level: leads program and educational initiatives, heavy involvement in student training and staff supervision; requires 3 years RN experience.
Who We’re Looking For
Minimum qualifications (by level)
- Entry
- Bachelor of Science in Nursing from an accredited program, or equivalent combination of education and technical experience on a year-for-year basis.
- Intermediate
- BSN from an accredited program and at least 1 year of professional RN experience.
- Senior
- BSN and at least 2 years of professional RN experience.
- Principal
- BSN and at least 3 years of professional RN experience.
Preferred
- Spanish bilingual strongly preferred.
- Experience providing care coordination in a Federally Qualified Health Center, public health setting, or serving low-income families.
- Case management certification and/or 2+ years of case management experience.
- Experience in program development, implementation, and supervising staff.
Required skills and competencies
- Strong interpersonal and communication skills for working with patients, families, interdisciplinary teams, and community partners.
- Proficiency with electronic medical records and clinic software.
- Ability to use independent judgment to monitor and respond to changes in patient conditions.
- Knowledge of HIPAA and OSHA compliance and Colorado immunization requirements.
- Strong organizational skills and ability to prioritize competing needs.
Location & Schedule
- Work location: Onsite at Confidential Client's community health clinic, Southwest Denver, CO (80236).
- Schedule: Full-time, 1.0 FTE (typical outpatient clinic schedule). This is not an inpatient bedside role.
Compensation & Benefits
Anticipated pay ranges by rank (good faith estimate):
- Entry Level: $60,257 to $70,500 per year
- Intermediate Level: $65,574 to $76,721 per year
- Senior Level: $70,890 to $82,942 per year
- Principal Level: $81,524 to $95,383 per year

Conditions of Employment & Compliance
- Must be legally authorized to work in the United States without sponsorship.
- Current RN licensure in good standing required; candidates may register with state regulatory agencies while completing licensure processes but must hold Colorado RN licensure by date of hire.
- Must obtain and maintain BLS/CPR certification by date of hire.
- Must have a valid National Provider Identifier (NPI) number by date of hire and maintain credentialing.
- Must be willing and able to work in a patient care environment with potential exposure to biological hazards and infectious diseases.
- Must be able to pass a background check.
