Physical Therapist - Per Diem
The Physical Therapist provides direct patient care by evaluating and treating patients based on referrals, developing and implementing individualized plans of care, and coordinating discharge planning with a multidisciplinary team. This role also involves supervising support staff, participating in quality improvement initiatives, mentoring new staff and students, and ensuring comprehensive documentation and communication to optimize patient functional outcomes in an inpatient rehabilitation setting.

What You’ll Do
Confidential Client is seeking a licensed Physical Therapist (Per Diem) to provide high-quality inpatient rehabilitation care. In this role you will assess patients, develop and implement individualized plans of care, provide direct treatment, and support safe discharges in collaboration with the multidisciplinary team.
- Evaluate patients’ physical abilities and baseline function after provider referral
- Develop and execute individualized plans of care with appropriate treatment techniques
- Reassess and update plans of care based on patient progress and clinical judgment
- Provide patient and caregiver education and discharge planning recommendations
- Document evaluations, progress, discharge plans, and communications in the medical record
- Communicate effectively with physicians, nurses, case managers and other team members
- Supervise and coordinate patient assignments for PTAs and rehabilitation technicians as needed
- Participate in quality-improvement activities, team meetings, and departmental initiatives
Responsibilities & Scope
- Direct patient care (approximately 90–100% of time): evaluation, treatment, documentation, and re-evaluation
- Indirect care (approximately 1–10%): department functions, supervision of support staff, and occasional float coverage to other care sites
- Education & mentoring: orient new staff, serve as a clinical instructor for PT/PTA students, and represent the discipline in committee meetings
- Maintain productivity and quality expectations consistent with departmental standards
- Adhere to High Reliability Organization (HRO) principles focused on safety, quality and efficiency
Minimum Qualifications
- Physical Therapist license in the State of Ohio (required)
- Bachelor’s degree (required); entry-level new graduates accepted if a veteran PT is available to mentor; otherwise a minimum of one year clinical experience is required
- Basic computer/documentation skills
- Driver’s license only required if role includes home care visits
Preferred / Nice-to-Have
- Experience in inpatient rehabilitation settings
- Prior supervision or preceptor experience
- Familiarity with multidisciplinary discharge planning and durable medical equipment (DME) recommendations

- Location: New Rome, OH (inpatient rehabilitation)
- Work shift: Day
- Scheduled weekly hours: As needed / Per diem (PRN)
- Estimated salary (from source data): $74,766 per year (estimated)
