What You’ll Do
Confidential Client is seeking a Full‑Time Outpatient Physical Therapist focused on Neuro Rehabilitation to provide patient-centered care in an ambulatory setting. You will evaluate patients, develop and carry out evidence-based plans of care, document clinical progress, and collaborate with the multidisciplinary team to maximize functional outcomes.
Key Responsibilities
- Provide direct patient care (approximately 90–100%): evaluate, treat, and reassess patients after referral.
- Develop individualized plans of care based on assessment findings and best-practice neuro rehab techniques.
- Deliver skilled therapeutic interventions and modify treatment plans as patients progress.
- Supervise and collaborate with physical therapist assistants and rehabilitation technicians.
- Coordinate discharge planning and recommend durable medical equipment as needed.
- Educate patients and caregivers on home programs, safety, and follow-up care.
- Maintain timely, accurate documentation in the medical record and communicate with physicians, case managers, and other team members.
- Participate in department quality improvement, staff meetings, and committees.
- Serve as a clinical instructor and mentor for PT and PTA students and assist with onboarding new staff.
Minimum Qualifications
- Bachelor’s or entry-level professional degree in Physical Therapy (required).
- Current Physical Therapy license in the state of Ohio (or ability to obtain prior to start).
- CPR certification recognized by the American Heart Association.
- Basic computer literacy for documentation and clinical reporting.
- Entry-level candidates may be considered if there is an experienced PT mentor on-site; otherwise, 1+ year of clinical experience preferred.
Preferred / Nice to Have
- Clinical experience in outpatient neuro rehabilitation.
- Experience supervising PTAs or serving as a clinical instructor.
- Strong communication and interdisciplinary collaboration skills.
- Commitment to patient education and evidence-based practice.
