Responsibilities

Confidential client is seeking a Home Health Physical Therapist (PT) to manage an assigned caseload and deliver high-quality, patient-centered rehabilitation in the home setting.

  • Manage daily patient caseloads and deliver timely, documented care per the established plan of care and physician orders
  • Collect and analyze assessment data from home visits, telephonic contacts, and the EMR to identify needs, risks, and appropriate interventions
  • Identify high-risk patients (re-hospitalization, falls, high resource use) and coordinate case conferencing or additional interventions
  • Prioritize patient issues based on clinical criticality and safety to ensure timely services
  • Evaluate patients to determine rehabilitation needs and potential, then develop and implement individualized plans of care
  • Provide PT services authorized by the physician and coordinate multidisciplinary care, including supervision of LPTAs and HHAs where applicable
  • Maintain ongoing communication with physicians, multidisciplinary team members, and branch supervision regarding patient status and plan-of-care changes
  • Document goals and progress in visit notes, monthly progress notes, and discharge summaries
  • Deliver treatments in alignment with State Physical Therapy Practice Act, Home Health Conditions of Participation, Medicare/OASIS, and agency policies

Minimum Qualifications

  • Doctor of Physical Therapy (DPT) degree required (or equivalent credential consistent with applicable state licensure)
  • Current Physical Therapy license in the state where work is performed (must obtain licensure prior to practice if not already licensed)
  • Basic Life Support (BLS) for Healthcare Provider certification (American Heart Association or equivalent)
  • Valid driver’s license, reliable insured vehicle, and ability to travel to patient residences
  • Strong clinical judgment, documentation skills, and ability to work independently in home settings
  • Demonstrated ability to communicate effectively with patients, families, and multidisciplinary teams

Preferred Qualifications

  • 1 year of experience as a licensed PT in a clinical or home health setting preferred
  • Pediatric experience (1 year within past 5 years) preferred — training may be provided by the confidential client

Physical Requirements & Working Conditions

  • Ability to walk, stand, sit, lift (support up to 50 lbs), reach, stoop, bend, push, and pull as required for patient handling and equipment
  • Ability to travel extensively to patient homes in all seasons and work a flexible schedule to meet patient needs
  • Work occurs in varied home environments; use of personal protective equipment (gloves, gowns, masks, eye protection) may be required
  • Must have intact vision, hearing, dexterity, and the ability to communicate in English effectively

Compensation & Schedule

  • Employment type: Full-time
  • Estimated compensation: $126,035 per year — estimated value provided by source
  • Location: Lincolnton, NC (work performed in patients’ homes throughout the area)
