What You’ll Do
Confidential Client is seeking a licensed Physical Therapist (PT) for Home Health care. As a key clinician you will manage an assigned caseload, deliver patient-centered therapy in the home setting, and coordinate care with the interdisciplinary team to achieve safe, measurable outcomes.
- Manage daily patient caseload and visits to meet the established plan of care and physician orders
- Perform comprehensive home assessments and collect data from in-person visits, telephonic contacts, and the EMR
- Develop, implement, and update individualized plans of care with short- and long-term, measurable goals
- Identify high-risk patients (re-hospitalization, falls, high resource utilization) and coordinate interventions to mitigate risk
- Supervise and collaborate with LPTAs and home health aides as applicable
- Document visit notes, monthly progress, and discharge summaries in compliance with Medicare, OASIS, and agency requirements
- Communicate care changes and patient status with physicians, case managers, and branch leadership
- Practice in accordance with the State Physical Therapy Practice Act and applicable accreditation and regulatory guidelines
Minimum Qualifications
- Doctor of Physical Therapy (DPT) degree required (or equivalent per current state licensure rules)
- Current Physical Therapy license for the state of practice (North Carolina) — must be licensed in the state where you work
- Basic Life Support (BLS) for Healthcare Providers from the American Heart Association
- Valid driver’s license, active auto insurance, and reliable personal vehicle for home visits
- Ability to lift and support up to 50 pounds and perform physical tasks required for patient care
- Strong clinical judgment, documentation skills, and the ability to work independently in patients’ homes
Preferred Qualifications
- 1 year of experience as a licensed PT in a clinical or home health setting preferred
- Pediatric experience within the last 5 years is a plus (training may be provided)
- Experience with Medicare/OASIS documentation, discharge planning, and case conferencing
Working Conditions & Physical Requirements
- Regular travel to patient residences; work in varied home environments and weather conditions
- Frequent walking, standing, lifting, bending, reaching, and transferring patients/equipment
- Use of personal protective equipment when required (gloves, masks, gowns, etc.)
- Ability to communicate clearly in English with patients, families, and the clinical team
