What You’ll Do

This role supports home-based family services and community outreach to strengthen parent–child relationships and preserve the medical home for at-risk families. Key responsibilities include:

• Provide home-based parenting education and developmental assessments.
• Deliver emotional support and create a safe, trusting environment for families.
• Provide referrals and resources based on family needs and follow up on referrals.
• Assist with monthly billing documentation, data collection, and clerical duties (data entry for home-visiting program).
• Perform initial assessments and manage referrals/screenings for program admittance; use tools such as the Family Stress Checklist.
• Coordinate playgroups and community support/education groups.
• Assist with client satisfaction calls and home observations.
• Build and maintain community relationships and collaborate with the Program Manager/Supervisor on referrals.
• Support staff training, orientation, and coordinate educational opportunities.
• Report safety incidents promptly, attend safety training, and ensure regulatory compliance.

Qualifications (Required)

• Associate’s Degree in Nursing, Social Work, or a related health field required; Bachelor’s degree preferred.
• Minimum 3 years’ experience working in child development or family support roles.
• Comfortable working in home-based settings and with community outreach.
• Proficient with common computer tasks and recordkeeping.
• BLS certification (American Heart Association) required.
• Valid Arizona driver’s license required.
• Fingerprint Clearance Card: application required upon hire; card must be obtained within 90 days.

Preferred / Helpful Skills

• Prior home-visiting program experience.
• Bilingual skills (Spanish or other languages) are a plus.
• Experience in healthcare or social work environments.
• Strong interpersonal skills, cultural sensitivity, and ability to build trust quickly.
