What You’ll Do

Confidential Client is hiring an Orthopedic Physical Therapist to deliver high-quality, home-based orthopedic rehabilitation to patients in Keyport, NJ. You will design and deliver individualized treatment plans that improve mobility, reduce pain, and restore function.

• Conduct comprehensive patient evaluations and functional assessments
• Develop, implement, and adjust evidence-based orthopedic treatment plans
• Provide skilled manual and therapeutic interventions in patients’ homes
• Document evaluations, treatment plans, and progress accurately in EHRs and comply with legal/regulatory documentation standards
• Coordinate with physicians, nurses, and interdisciplinary team members to ensure continuity of care
• Educate patients and caregivers on home exercise programs and injury prevention
• Supervise and mentor support staff when assigned
• Participate in community outreach and staff development initiatives

Work Environment & Schedule

This is a full-time (40 hours/week) home care role based in Keyport, New Jersey. The schedule may include variable weekends to meet patient needs. You’ll be working directly in patients’ homes across the service area; travel and reliable transportation are required.

Minimum Qualifications

• Bachelor’s degree in Physical Therapy required; Doctor of Physical Therapy (DPT) strongly preferred
• Current, unrestricted Physical Therapist license in New Jersey
• Strong written and verbal communication skills
• Proficiency with electronic health record (EHR) documentation
• Ability to work collaboratively across interdisciplinary care teams
• Must be eligible to work in the United States

Preferred Experience

• Previous experience in orthopedic rehabilitation and post-surgical care
• Experience delivering home health or home-care physical therapy
• Familiarity with community outreach and patient education programs

Compensation & Benefits

• Base salary: $100,000 – $120,000 per year (commensurate with experience)
