What You’ll Do
This entry-level Office of Aging Case Manager will provide assessment and care‑management services that help older adults remain independent, safe, and engaged in their community. You will conduct comprehensive assessments, develop individualized care plans, coordinate services, and monitor progress — working with clients, families, and community partners.
- Conduct psychosocial and functional assessments for older adults
- Develop person-centered care plans addressing ADLs, supports, cognition, health and economic resources
- Coordinate referrals to community supports, health services, and benefits programs
- Monitor client progress and update care plans as needs change
- Maintain accurate case records and follow agency policies and procedures
- Use judgment to escalate complex cases and collaborate with supervisors and multidisciplinary teams
Key Responsibilities
- Provide direct care management for older adults with functional disabilities
- Evaluate social supports, daily living abilities, cognitive status, physical health, and resources
- Create and implement individualized care plans aimed at maintaining independence
- Communicate effectively with clients, caregivers, service providers, and community partners
- Complete required documentation and reports in a timely manner
- Participate in team meetings and trainings to improve service quality
Essential Qualifications
- Bachelor’s degree in Social Work or a related field
- Minimum 1 year of social work experience involving comprehensive functional and/or psychosocial assessments
- Comfortable operating PC hardware in networked environments and using Microsoft Office (Word, Access, Excel)
- Valid driver’s license, reliable daily transportation, and appropriate insurance
- Ability to travel out-of-town and overnight for agency purposes when required
- Ability to perform essential job functions: sit, stand, climb stairs, lift/carry up to 20 lbs safely, and access homes that may be inaccessible
Required Clearances & Background Checks
To comply with program and state requirements, applicants must obtain (or have current) the following prior to employment:
- Pennsylvania State Police Criminal History Clearance
- Child Abuse Clearance
- FBI Clearance and National Sex Offender Registry check (current within 90 days of employment)
- Motor Vehicle Record check
Compensation & Employment Details
- Pay rate: $21.06 per hour (approx. $33,657/year)
- Employment type: Full‑time
- Location: Lock Haven, PA — local travel required for home visits and occasional overnight travel for agency purposes
