What You’ll Do
This entry-level Care Manager I role delivers person-centered assessment and care management services that help older adults with functional disabilities remain independent, safe, and connected to their communities. Responsibilities include:
- Conduct comprehensive functional and psychosocial assessments for older adults (activities of daily living, cognitive and physical health, social supports, economic resources).
- Develop individualized care plans based on assessment findings and community resources.
- Monitor progress, update care plans, and coordinate services with community partners and providers.
- Maintain accurate records and documentation according to established policies and procedures.
- Use sound professional judgment when handling moderately complex cases and escalate as needed.
- Communicate clearly with clients, families, and caregivers; provide referrals and follow-up support.
Compensation
- PAY RATE: $21.06 per hour (estimated annual pay ≈ $43,275).
Minimum Qualifications
- Bachelor’s degree in Social Work or a related field.
- At least one year of experience in public or private social work involving comprehensive functional and/or psychosocial assessments.
- One year of experience using PC hardware in a networked environment and Microsoft Office products (Word, Access, Excel).
- Clear speaking voice and the mental/physical ability to perform essential job functions (sit, stand, climb stairs, lift up to 20 lbs., access non‑accessible residences).
- Valid driver’s license, reliable daily transportation, and appropriate insurance.
Required Clearances & Other Requirements
- Pennsylvania State Police Criminal History Clearance, Child Abuse Clearance, FBI Clearance, and National Sex Offender Registry check current within 90 days of employment.
- Motor Vehicle Record check.
- Ability to travel out-of-town and overnight for agency purposes when required.
