About the Role

Confidential Client is hiring an entry-level Office of Aging Care Manager I to provide person-centered assessment and care management services that help older adults remain independent, safe, and connected to their communities. This role focuses on conducting functional and psychosocial assessments, developing individualized care plans, coordinating services and community resources, and documenting progress according to agency policy.


Key Responsibilities

  • Conduct comprehensive functional and psychosocial assessments for older adults.
  • Develop individualized care plans that address activities of daily living, cognitive and physical functioning, social supports, economic resources, and safety needs.
  • Coordinate and refer clients to community resources, home- and community-based services, and health or social service providers.
  • Monitor client status and update care plans as needs change.
  • Maintain accurate, timely records and documentation in agency systems.
  • Communicate effectively with clients, family members, caregivers, and community partners.
  • Follow established practices and procedures; escalate complex situations as needed.
  • Travel to client homes and community locations as required.

Qualifications

  • Bachelor’s degree in Social Work or a related field.
  • Minimum of one year of experience in public or private social work involving comprehensive functional and/or psychosocial assessments.
  • Proficiency using personal computers in a networked environment and Microsoft Office (Word, Access, Excel).
  • Valid driver’s license, reliable transportation, and appropriate insurance coverage.
  • Ability to travel out-of-town and overnight for agency purposes when required.
  • Clear speaking voice and the mental/physical ability to perform essential duties, including sitting, standing, climbing stairs, lifting and carrying up to 20 pounds unassisted, and accessing residences that may not be wheelchair accessible.
  • Required background clearances and checks (see Requirements below).

Requirements / Pre-employment Clearances

  • Pennsylvania State Police Criminal History Clearance, Child Abuse Clearance, FBI Clearance, and National Sex Offender Registry check current within 90 days of employment.
  • Motor Vehicle Record check.
  • Ability to meet the physical demands listed in the Qualifications section.

Compensation & Schedule

  • Pay rate: $21.06 per hour (estimated annual equivalent shown below).
  • Estimated annual: $45,796 per year (based on full-time hours).
  • Employment type: Full-time.

Work Location

  • Primary location: Lock Haven, PA.
  • This role requires travel to client homes and occasional out-of-town/overnight travel for agency business.
