Work Location

Hybrid / remote with required in-person community visits across Pennsylvania. Candidates must reside in one of the following counties to complete home visits: Philadelphia, Delaware, Montgomery, Chester, or Bucks County.

Compensation

• $20 - $21 per hour (starting hourly rate)
• Mileage reimbursement for work-related travel
• Paid training period; tuition reimbursement eligibility after one year

Work Environment Requirements

This is a hybrid position requiring both in-office and remote work. When working remotely, employees must maintain reliable high-speed internet access for meetings, systems, and communication. Failure to maintain adequate internet may impact the ability to perform essential job functions.

The role includes regular in-community visits; frequent exposure to outdoor elements and occasional driving in poor weather conditions may be required.

Education & Experience

• Associate’s or Bachelor’s degree in Social Work, Human Services, Nursing, Psychology, Sociology, or related field required (Bachelor’s degree strongly preferred)
• Recent college graduates are encouraged to apply
• Previous experience in care coordination, case management, healthcare, or human services preferred but not required for entry-level candidates
• Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS) is a plus

Position Overview

The Nursing Home Transition (NHT) Coordinator supports individuals transitioning from nursing facilities to community-based living. You will coordinate with participants, families, service coordinators, healthcare providers, housing resources, and community agencies to develop and implement transition plans that ensure safe, sustainable discharges and community integration.

This role requires accurate, timely documentation and close adherence to program and regulatory requirements. The Confidential Client welcomes both experienced professionals and recent graduates and provides training and mentorship to build capabilities in care coordination and participant-centered service delivery.

Key Responsibilities

• Serve as the primary contact for nursing home residents transitioning into community-based living through the NHT program
• Coordinate transition plans with participants, families, healthcare teams, housing providers, and community agencies
• Arrange housing and essential support services to ensure successful discharge and reintegration
• Monitor participant progress and maintain accurate documentation in compliance with regulatory and organizational standards
• Support outreach, quality assurance initiatives, mentorship activities, and regional travel as needed

Skills & Knowledge

• Experience working with individuals with disabilities or special needs preferred
• Strong assessment, planning, and participant-centered service delivery skills
• Excellent organizational, verbal, and written communication skills, with attention to documentation and compliance
• Ability to work effectively on an interdisciplinary team
• Familiarity with community support systems, housing resources, healthcare, and human services preferred
• Willingness to learn and grow within the organization
