What You’ll Do

  • Serve as the primary contact for nursing home residents transitioning to community-based living through the Nursing Home Transition (NHT) program.
  • Assess participant needs and develop participant-centered transition plans with individuals, families, healthcare teams, housing providers, and community agencies.
  • Arrange housing, services, and supports necessary for successful discharge and community reintegration.
  • Coordinate with service coordinators, Medicaid/HCBS resources, and external providers to ensure continuity of care.
  • Monitor participant progress, maintain timely and accurate documentation, and ensure compliance with program and regulatory requirements.
  • Provide outreach, support quality assurance initiatives, participate in mentorship activities, and travel regionally for home visits and community meetings as needed.

Qualifications

  • Associate’s or Bachelor’s degree in Social Work, Human Services, Nursing, Psychology, Sociology, or a related field required (Bachelor’s strongly preferred).
  • Recent college graduates are encouraged to apply; comprehensive on-the-job training and mentorship provided.
  • Previous experience in care coordination, case management, healthcare, or human services preferred but not required for entry-level candidates.
  • Knowledge of Medicaid Waiver programs, Long-Term Services & Supports (LTSS), and Home and Community-Based Services (HCBS) is a plus.
  • Valid driver’s license and reliable transportation for community/home visits may be required.

Skills & Knowledge

  • Strong participant-centered assessment, planning, and case coordination skills.
  • Excellent organization, written and verbal communication, and documentation skills.
  • Ability to work collaboratively with interdisciplinary teams and external community partners.
  • Familiarity with community support systems, housing resources, and healthcare service delivery.
  • Willingness to learn, adapt, and grow within a human services organization.

Compensation & Benefits

  • Starting Hourly Rate: $21.00 / hour (with opportunities for performance-based increases).
  • 15 Agency Paid Days Off (includes 11 holidays + 4 self-care days).
  • 18 PTO days.
  • Flexible schedule; no on-call requirements.
  • Paid training period and mileage reimbursement for business travel.
  • Medical insurance, company-paid dental and vision.
  • Company-paid basic life insurance (option to add additional coverage).
  • Up to 5% employer match for 401(k).
  • Tuition reimbursement eligibility after one year to support continued education and career growth.

Work Location & Schedule

  • Hybrid/remote position requiring both in-office and remote work plus in-person visits across Pennsylvania.
  • Candidates must reside in one of the following counties to complete home visits: Philadelphia, Delaware, Montgomery, Chester, or Bucks County.
  • When working remotely, employees must maintain reliable high-speed internet to support meetings and systems access.
  • Frequent outdoor exposure and occasional driving in varying weather conditions may be required.
