What You’ll Do

Confidential Client is hiring Mental Health Case Managers to provide community-based support for adults experiencing severe and persistent mental illness. This role combines office, community, and telehealth work to develop individualized recovery plans and coordinate supports that help clients live healthy, productive lives.

• Provide Community Psychiatric Support Treatment (CPST) and Therapeutic Behavioral Services (TBS) in outpatient and community settings
• Conduct assessments, develop Individual Recovery Plans, and coordinate access to medical care, housing, employment, and other supports
• Deliver face-to-face and telehealth sessions and respond to crisis needs as required
• Establish therapeutic relationships and use problem-solving interventions to support client functioning
• Collaborate with other providers, delegate recovery plan tasks appropriately, and monitor service delivery
• Serve clients across the Greater Cincinnati area, including Hamilton and Clermont counties

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, Criminal Justice, or related Human Services field

• Strong communication, organizational, and client-engagement skills

• Estimated base pay: $38,818 per year (estimated)
