Role & Compensation
Confidential Client is hiring a Mental Health Case Manager to provide community-based case management for adults with serious and persistent mental illness. This is a Contract / Full-time role based in Minnetonka Mills, MN with a hybrid work model.
- Compensation: $50,000 per year (50000–50000 yearly)
- Employment type: Contract / Full-time
- Work model: Hybrid (mix of remote and in-office); regular community visits required

What You’ll Do
As a Mental Health Case Manager for Confidential Client you will:
- Provide direct client support — build trusting relationships and assist with housing, financial, vocational, educational, legal, and medical needs
- Coordinate services and referrals across agencies and providers to ensure holistic care
- Complete required documentation, including Functional Assessments, Crisis Plans, and Individual Community Support Plans (ICSPs), in compliance with Minnesota Rule 79 guidelines
- Deliver crisis intervention and short-term stabilization supports when needed
- Meet regularly with clients to monitor progress, adjust care plans, and advocate on their behalf
- Build and maintain community connections with county/state officials, families, employers, and community organizations
What You’ll Bring
- Bachelor’s or Master’s degree in Human Services, Psychology, Social Work, or related field — or qualification as a Mental Health Practitioner. Social Work candidates must hold current Social Work licensure.
- At least one year of professional or internship experience in the mental health field
- Valid Minnesota driver’s license and access to a reliable vehicle for community visits (mileage reimbursement provided)
- Passion for helping people with serious mental illness and strong community-based engagement skills
