Role Overview
The Confidential Client seeks a Member Engagement Coordinator to welcome Members into the network, complete intake assessments, and coordinate initial handoffs to Care Managers. This full-time, hourly role focuses on member education about care models (including ACOs), completing social determinants of health surveys, building rapport with Members and families, and supporting care coordination across platforms. This position is based in Temple Terrace, FL.
Key Responsibilities
- Provide an initial welcome call to Members and families and confirm network enrollment.
- Complete entry-level assessments and social determinants of health surveys for new Members and on a quarterly cadence.
- Explain the Accountable Care Organization (ACO) model and the reason for Members’ assignment.
- Educate Members and families on benefits, resources, and how to access support.
- Maintain and expand knowledge of high-need community resources and referral pathways.
- Coordinate smooth handoffs from the Engagement team to Care Managers and resolve initial concerns.
- Lead customer engagement surveys to monitor service quality across products.
- Perform other duties as assigned to support member experience and program goals.
Qualifications
- Bachelor’s degree in Human Services or a related field preferred.
- Minimum of 1 year in healthcare-related roles or equivalent experience working with patients/families.
- Strong verbal and written communication skills.
- Highly organized, self-motivated, and able to manage competing priorities.
- Knowledge of the care continuum and experience connecting patients to services via multiple channels.
- Bilingual in Spanish preferred.

Compensation & Conditions
- Pay range: $20.26 - $30.16 per hour.
- Employment type: Full-time, hourly.
