Role Overview
Confidential Client is seeking a Licensed Physical Therapist to provide one-on-one home care services in Lehigh Acres, FL. This full-time, contract position focuses on evaluating patients, developing individualized treatment plans, and delivering hands-on therapy in patients' homes. New graduates are welcome — mentorship and professional development are provided.
What You’ll Do
Confidential Client expects you to deliver compassionate, evidence-based physical therapy in a home-care setting while documenting progress and coordinating with caregivers and other clinicians.
- Conduct comprehensive patient evaluations and create individualized treatment plans
- Deliver therapeutic interventions including exercise programs, manual therapy, and use of assistive devices
- Set measurable short- and long-term goals with patients and caregivers
- Monitor patient progress and revise plans based on outcomes and feedback
- Provide caregiver/patient education on safe mobility and equipment use
- Maintain accurate documentation of treatments, outcomes, and communications
- Participate in continuing education and team case reviews
Key Qualifications
Applicants must meet all legal requirements to practice in Florida and should demonstrate the skills below.
- Licensed Physical Therapist with an active Florida PT license
- Bachelor's degree in Physical Therapy (or equivalent from an accredited program)
- Strong written and verbal communication skills
- Comfortable using electronic documentation and general office software
- Flexibility to adapt treatment to varied patient needs and environments
- Open to new graduates — clinical internships or home-care rotations are a plus
Preferred Experience
- Clinical rotations or internships in home health, pediatrics, orthopedics, or geriatrics
- Prior experience delivering therapy in patient homes or community settings
- Experience with mobility assessments and assistive device recommendations

- Salary range: $100,000 – $120,000 per year
