What You’ll Do

Confidential Client is hiring a Field Registered Nurse (RN) to deliver skilled home health care across the TriValley / San Ramon area. This is a field role — you will visit patients in their homes and deliver hands-on clinical care under the direction of a Case Management RN.

• Provide hands-on skilled nursing (within RN scope): PICC management, wound vacs and wound care, feeding pumps (Kangaroo), NG/G/J tube management, staple/suture removal, drains (JP, biliary, nephrostomy), Foley/suprapubic catheter care (insert/remove/troubleshoot), PleurX management, trach care, blood draws, ostomy care, SQ/IM injections, IV antibiotic/TPN administration, and compression wraps.
• Implement and follow plans of care initiated by the Case Management RN.
• Accurately document care and chart within 24–48 hours per agency policy.
• Educate patients, families, and caregivers about diagnoses, treatments, and self-care.
• Coordinate with interdisciplinary team members (HHA, OT, PT, MSW, ST) and report needs to the Case Management RN.
• Prepare equipment and supplies, assist with procedures, and support patient learning of self-care techniques.
• Participate in weekly case conferences and on-call weekend rotation as assigned.
• Maintain patient privacy, professionalism, productivity, and ongoing clinical skill development.

Who You Are / Qualifications

• Active California Registered Nurse (RN) license — required.
• BLS certification — required.
• Valid California driver’s license and acceptable driving record.
• Graduate of an accredited school of nursing (BSN preferred).
• 1 year recent nursing experience preferred (requirement may be waived for internal candidates or new grads).
• Home health experience and high-acuity patient experience are a plus.
• Strong clinical judgment, critical thinking, and excellent verbal and written communication skills.
• Comfortable working independently in the field and collaborating with an interdisciplinary team.
• Computer literate and able to complete electronic documentation on schedule.
• All licenses and certifications must be current at time of hire and maintained throughout employment.

Schedule & Location

• Full-time, Wednesday–Sunday visit schedule.
• Coverage area: TriValley / San Ramon areas.
• This role requires regular in-person patient visits — telecommuting is not an option.

Compensation & Benefits

• Estimated salary: $130,000 - $160,000 per year (compensation follows a pay-per-visit model; starting pay is based on relevant experience).
