Position Overview

Confidential Client is seeking a Home Health Field Registered Nurse (RN) to provide skilled nursing care to patients in Solano County. This full-time field role follows a Wednesday–Sunday schedule and is an entry-level field RN role designed to build clinical skills and prepare nurses for advancement into case management. This position reports to a Case Management Registered Nurse and requires regular travel to patient homes.


What You’ll Do

  • Provide hands-on patient care within RN scope, including PICC line care, wound vacs, feeding pumps, NG/G/J tube management, staple/suture removal, and drain care (e.g., JP, biliary, nephrostomy).
  • Manage catheters (Foley/suprapubic) — insertion, removal, troubleshooting — and pleurX drainage systems.
  • Perform tracheostomy care, blood draws, ostomy management, SQ and IM injections, IV antibiotic/TPN administration, and compression wrap application.
  • Deliver direct patient care consistent with the state Nurse Practice Act and the plan of care.
  • Implement and document the plan of care initiated by the Case Management RN within 24–48 hours.
  • Educate patients, families, and caregivers on diagnosis, treatments, and self-care techniques.
  • Assist with skilled procedures and prepare equipment and supplies.
  • Coordinate with interdisciplinary team members (HHA, OT, PT, MSW, ST) and report identified needs to the Case Management RN.
  • Maintain accurate, timely clinical documentation and report incidents and significant changes promptly.
  • Attend weekly case conferences and participate in on-call weekend rotations as required.
  • Maintain productivity standards, professionalism, and pursue ongoing clinical development.

Qualifications

  • Current Registered Nurse (RN) license for California.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Current Basic Life Support (BLS) certification.
  • Valid California driver’s license and acceptable driving record.
  • Minimum: 1 year of recent nursing experience (may be waived for internal candidates or new grads); 1 year of home health experience preferred.
  • Experience with high-acuity patients and working on an interdisciplinary team is a plus.
  • Strong clinical judgment, critical thinking, organization, and communication skills.
  • Comfortable using electronic documentation systems and meeting required compliance training.
  • All licenses and certifications must be current at time of hire and maintained throughout employment.

Schedule & Location

  • Location: Fairfield, CA — coverage primarily Solano County. This is an in-field role; clinicians visit patients in their homes.
  • Schedule: Full-time, Wednesday–Sunday (visits).
  • On-call weekend rotation may be required.

Compensation & Benefits

  • Estimated base pay: $130,000 - $160,000 per year (pay-per-visit model; starting pay depends on relevant experience). This posting complies with applicable pay-transparency requirements for the state of California.
