Position Overview
Confidential Client is seeking a dedicated Home-Based Physical Therapist to deliver high-quality, patient-centered in-home care in Chantilly, VA and the surrounding communities. This full-time, remote-capable role focuses on improving mobility, reducing pain, and helping patients recover safely after injury or surgery. The ideal candidate is a licensed Physical Therapist with strong evaluation and treatment skills, excellent communication, and a commitment to measurable patient outcomes.
What You’ll Do
You will provide comprehensive in-home physical therapy services, building individualized plans that support each patient’s recovery goals.
- Conduct thorough initial evaluations and ongoing assessments to develop personalized treatment plans.
- Explain findings and treatment goals clearly to patients and caregivers; involve them in goal-setting.
- Deliver evidence-based interventions (therapeutic exercise, manual therapy, modalities as appropriate) and adjust plans based on progress.
- Complete timely, accurate clinical documentation (daily notes, progress reports, discharge summaries).
- Coordinate care with interdisciplinary team members and referring providers to ensure continuity.
- Manage your schedule efficiently and meet productivity expectations while maintaining quality care.
- Participate in community outreach and referral relationship-building activities.
- Engage in professional development and mentor students or newer staff when appropriate.
Who You Are (Required Qualifications)
- Bachelor’s or entry-level professional degree in Physical Therapy from an accredited program (or equivalent as required by state licensure).
- Current, unrestricted license to practice Physical Therapy in Virginia.
- Current BLS/CPR certification (AHA Healthcare Provider or equivalent).
- Strong clinical assessment, manual therapy, and therapeutic exercise skills.
- Excellent verbal and written communication skills; professional bedside manner.
- Reliable transportation and ability to travel to patient homes across Chantilly and nearby areas.
Preferred
- 2+ years of clinical experience, ideally including home health or in-home care.
- Experience with geriatric, post-surgical, or mobility-limited populations.
- Previous experience collaborating with case managers, home health aides, or skilled nursing settings.
Work Environment & Schedule
This is an in-home role (jobLocationType: TELECOMMUTE) serving patients primarily in Chantilly, VA and nearby communities. Schedules are generally Monday–Friday with some flexibility; travel between patient homes is required. Confidential Client supports a collaborative team culture and provides administrative support for scheduling and billing.
Compensation & Benefits
- Salary range: $80,000 - $100,000 per year (commensurate with experience).
