Work Environment
This is an onsite, home-care delivery role based in Bayonne, NJ. You will provide one-on-one care in patients' residences, delivering personalized rehabilitation services and collaborating closely with other healthcare professionals to support patient outcomes.
Key Responsibilities
- Conduct thorough patient evaluations using relevant history and targeted assessments to develop individualized treatment plans.
- Communicate evaluation findings and treatment plans to patients and their families; set measurable goals to promote adherence.
- Implement and adapt therapeutic interventions for foot, ankle, and lower-limb conditions based on ongoing assessment and patient response.
- Coordinate care with a multidisciplinary team to ensure comprehensive, patient-centered treatment.
- Maintain accurate, timely documentation of patient progress and treatment outcomes in compliance with applicable healthcare regulations.
Patient Population / Client Focus
You will primarily work with patients recovering from foot and ankle surgeries, individuals with chronic conditions (e.g., arthritis, diabetes) affecting lower-limb function, and those with sports-related injuries. The role centers on improving mobility, reducing pain, and restoring function through evidence-based rehabilitation.
Qualifications
- Bachelor’s degree in Physical Therapy or a related field (or equivalent professional degree as required by state licensure).
- Valid license to practice as a Physical Therapist in New Jersey.
- Strong assessment, clinical reasoning, and documentation skills.
- Excellent verbal and written communication; ability to engage patients and family members in care plans.
- Ability to provide care in patients’ homes and travel safely throughout assigned service area.
Preferred Experience
- Experience in outpatient or home-based physical therapy with a focus on lower-extremity and foot/ankle rehabilitation.
- Familiarity with home care delivery models and interdisciplinary care coordination.

Compensation and Benefits
