The Facilitator coordinates Child and Family Teams to develop and implement individualized service plans, providing case management and direct support to children and families. Role overview
Confidential Client is seeking a Facilitator to coordinate Child and Family Teams (CFTs), support development and implementation of individualized service plans, provide case management, and deliver direct services to children and families as needed. This role reports to a Program Supervisor and is based in Victorville, CA. The position is available as full-time or contractor.
What You’ll Do
- Coordinate, schedule, and facilitate Child and Family Team meetings to develop individualized service plans.
- Manage implementation of Child and Family Plans and coordinate delivery of formal and informal services.
- Provide case management, maintain timely documentation (contact forms, CANS, progress notes, service summaries, mileage, data tracking).
- Engage and build professional, boundary-respecting relationships with children, families, and collateral partners.
- Serve as liaison with community partners (mental health coordinators, county social workers, probation officers, schools).
- Provide crisis stabilization and support when plans disrupt; participate in after-hours on-call when required.
- Monitor approved plan expenses and FlexFunds when applicable.
- Maintain expected client-contact/service percentage standards; complete other duties as assigned.
Minimum qualifications
- Bachelor’s degree in Social Work, Psychology, Marriage and Family Therapy, or closely related behavioral science field AND two years’ experience providing services to children and families in a mental health or social services setting — OR —
- Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or closely related behavioral science field.
Preferred qualifications
- Completion of a clinical Master’s program.
- Clinical license or current intern registration consistent with state standards (or license-eligible/waiver status).
- Experience in crisis intervention and stabilization.
- Supervisory or coaching experience and knowledge of modern management practices.
- Bilingual in Spanish is a plus.
Position & program requirements
- Valid California driver’s license, personal automobile insurance, and a driving record that meets agency standards.
- Must be willing to complete TB testing, drug screening, and a state-conducted background investigation.
- Must obtain First Aid certification (and CPR where required by county/program).
- Flexible to work varying schedules according to operational needs.
- Must work respectfully in a culturally integrated workplace and comply with federal, state, and local protected-class nondiscrimination laws.
Physical requirements
- Ability to work in residential, office, and outdoor environments: standing, walking (even and uneven surfaces), bending, kneeling, twisting, reaching, balancing.
- Occasional lifting up to 25 pounds; frequent lifting up to 10 pounds.
- Able to walk stairs routinely and perform CPR/First Aid as trained.
- Able to sit for extended periods while traveling (vehicle travel may be up to 100 miles).
- Able to use computer and telephone systems; fine motor skills for typing and paperwork.
Compensation & benefits
- Pay range: $28.44 per hour — $39.11 per hour (actual pay depends on verified experience).
- Additional $1.00–$3.00 per hour for education.
- Additional $1.92 per hour for bilingual Spanish skill.
