Confidential Client — Alameda, CA (community-based; frequent travel throughout Alameda County).

Job Summary

Under the direct supervision of a Program Supervisor, the Facilitator coordinates Child and Family Teams (CFTs) and supports the development and implementation of individualized service plans. When applicable, the Facilitator provides direct services to children and families to maximize engagement and ensure implementation of plans.


Essential Functions

  • Coordinate, schedule, and facilitate Child and Family Team meetings to develop individualized service plans.
  • Oversee implementation of Child and Family Plans and provide case management, coordinating formal and informal resources.
  • Build effective, boundaries-aware relationships with children, families, and community partners.
  • Manage a flexible schedule to meet program needs, including travel throughout Alameda County and occasional work from home.
  • Complete required documentation on time (client contact forms, CANS, progress notes, data tracking, mileage, service activity summaries).
  • Assist in monitoring plan-approved expenses, budgets, and FlexFunds when applicable.
  • Serve as liaison with community agencies (County Social Workers, Probation, school staff, mental health coordinators) to coordinate services.
  • Provide crisis stabilization and support; participate in after-hours on-call when required by program leadership.
  • Meet agency service percentage standards for client-facing time, if applicable.
  • Perform other duties as assigned.

Minimum Qualifications

  • Bachelor’s degree in Social Work, Psychology, Marriage and Family Therapy, or closely related behavioral science AND two years’ experience providing services to children and families in mental health or social services — OR — a Master’s degree in one of these fields.

Preferred Qualifications

  • Completion of a clinical Master’s program.
  • Clinical license or current intern registration consistent with state standards (or license-eligible/waiver status).
  • Experience in crisis intervention and stabilization.
  • Supervisory or coaching experience and familiarity with modern management practices.

Position & Program Requirements

  • Valid California driver’s license, personal automobile insurance, and a driving record that meets agency standards.
  • Willingness to complete TB testing, drug screening, and state background checks as required.
  • Obtain First Aid Certification and CPR certification when required by county/program.
  • Flexibility to work a varying schedule according to operational needs.
  • Ability to work in a culturally integrated environment and respect differences protected by law.

Physical Requirements

  • Ability to work in residential, office, and outdoor environments: standing, walking (even/uneven surfaces), bending, kneeling, twisting, reaching, balancing, and occasional lifting up to 25 lbs.
  • Able to walk stairs routinely and sit for prolonged vehicle travel (may travel up to ~100 miles for visits).
  • Ability to perform CPR/First Aid as trained.
  • Proficient with computer and phone systems for documentation and communication.

Compensation & Benefits

  • Pay range: $28.44 - $39.11 per hour (final pay depends on verified experience).
  • Additional $1.92 per hour for bilingual Spanish speakers.
