About the Role
Confidential client is seeking a community-focused Consumer Services Coordinator to provide information, advocacy, and service arrangement for consumers and families. This is a part-time, community-based role centered on case coordination, person-centered planning, and connection to services for individuals with intellectual and developmental disabilities.
What You’ll Do
- Participate in development, monitoring, and revision of individualized program plans.
- Identify, coordinate, and arrange services for individuals and their families.
- Complete documentation, forms, and reports in accordance with regulations and organizational policies.
- Serve as an advocate for individuals with community agencies and service providers.
- Schedule and attend interdisciplinary meetings with individuals, family members, service providers, and advocates.
- Identify barriers to service delivery and implement culturally sensitive strategies that promote empathy, trust, and respect.
- Rotate officer-of-the-day duties; assist colleagues with special projects and case coverage; provide emergency on-call services as required.
- Work both in-office and in community settings (including home visits) to meet consumer needs.
Responsibilities & Impact
- Ensure timely, accurate documentation and follow-up on service plans.
- Build trusting relationships with consumers and families to support goal achievement.
- Coordinate across multiple providers and community resources to reduce service gaps.
- Apply person-centered planning approaches to empower consumers.
- Escalate and document complex cases, ensuring compliance with applicable regulations.
Minimum Qualifications
- Bachelor’s degree in psychology, social work, sociology, or a related human services field AND at least 2 years of related experience. A Master’s degree in a related field may be substituted for experience.
- Experience working with individuals with intellectual and developmental disabilities preferred.
- Proficiency with MS Word and MS Outlook.

- Strong organizational skills and ability to work independently and as part of an interdisciplinary team.
- Excellent verbal and written communication skills.
- Ability to work comfortably in underserved and non-traditional office settings.
- Cultural sensitivity and the ability to build rapport with diverse populations.
- Bilingual Spanish may be required for some positions.
Essential Requirements
- Community-based work: frequent in-person meetings in homes and community settings.
- Comfortable working with a high degree of independence and managing a varied schedule (telephone, virtual, and out-of-office meetings).
- Valid California Driver’s License and reliable transportation (or acceptable substitute) required.
- Part-time employment; benefit costs are pro-rated.
Compensation & Benefits
- Employment type: Part-time, non-exempt.
- Regular salary range: $26.86 - $37.96 per hour.
- Typical starting pay: $26.86 - $29.65 per hour depending on experience and internal equity.
