What You’ll Do

Confidential Client is seeking a Community-Facing Consumer Services Coordinator to provide information, advocacy, and service coordination for individuals and families. This role focuses on person-centered planning and community-based service delivery.

  • Participate in development, monitoring, and revision of individualized program plans
  • Identify and coordinate services and community supports for individuals and families
  • Complete documentation, forms, and required reports in accordance with regulations and organizational policies
  • Advocate for individuals served with community agencies and service providers
  • Schedule and attend interdisciplinary meetings with consumers, family members, providers, and advocates
  • Identify and address barriers to service delivery using culturally sensitive approaches
  • Rotate officer-of-the-day duties; assist colleagues with special projects and provide case coverage and on-call emergency support as needed

Qualifications

  • Bachelor’s degree in psychology, social work, sociology, or a related human services field AND at least 2 years of related experience; OR
  • Master’s degree in a related field may substitute for required experience

Skills & Abilities

  • Understanding of family systems and commitment to person-centered planning
  • Knowledge of intellectual and developmental disabilities and related community supports
  • Strong organizational skills and the ability to work independently and as part of an interdisciplinary team
  • Excellent verbal and written communication skills
  • Proficiency with MS Word and MS Outlook
  • Comfort working in community and non-traditional office settings to meet consumer needs
  • Bilingual Spanish may be required for some positions (preferred depending on assignment)

Compensation & Benefits

  • Hourly pay range: $26.86 - $37.96 per hour (non-exempt)
  • Typical starting salary: $26.86 - $29.65 per hour, depending on experience and internal equity

Work Environment & Requirements

  • Community-based role requiring travel to homes, schools, and community locations
  • Frequent phone, virtual, and in-person meetings
  • Valid California driver’s license and reliable transportation, or acceptable substitute, required
  • Ability to work independently with a high degree of responsibility and confidentiality
