What You’ll Do
Confidential Client is hiring a Community Based Case Manager to deliver community- and home-based skill-building and behavioral health support for clients (ages 5+). This role focuses on practical, recovery-oriented interventions that increase independence, stabilize behavior, and connect clients to community resources.
- Provide weekly community-based and in-home skill-building sessions
- Support clients with behavioral health needs, emotional disturbance, or chronic mental illness
- Teach coping strategies, daily living & life skills, and independence-building techniques
- Link clients to local resources, supports, and referral services
- Complete timely, accurate documentation: progress notes, treatment plan updates, and discharge summaries
- Collaborate with supervisors, clinicians, and community partners to coordinate care
Who We’re Looking For (Qualifications)
- Bachelor’s degree preferred (Human Services fields such as Social Work, Psychology, Counseling, Sociology, Nursing, OT/PT, Behavioral Sciences, etc.) — open to entry-level / recent graduates
- Ability to pass background checks, including Enhanced Fingerprinting with Idaho DHW
- Valid driver’s license, reliable transportation, and proof of auto insurance (required for community-based work)
- Comfortable using electronic health records (EHR) — experience is a plus
- Strong communication, documentation, and client-focused skills
Compensation & Benefits
- Pay: $20–$23 per hour (based on experience)
- Flexible schedule you design; part-time start with potential to grow to full-time
- Incentives for meeting/exceeding 22–32 client hours per week
