What You’ll Do
Confidential Client is seeking a full-time Case Manager (TSS) to support clients in recovery within our residential services team. You will work under the Program Director/Clinical Supervisor to provide case management, crisis support, and recovery-focused services that promote client safety, independence, and long-term recovery.
- Provide advocacy and case management services for assigned residents
- Build strong, supportive relationships and assess client needs regularly
- Complete clinical documentation: assessments, individual action plans, group & weekly progress notes, discharge summaries, and aftercare planning
- Facilitate evidence-based psychoeducation and recovery groups
- Deliver weekly one-on-one case management sessions
- Administer and document EIM/ESM and funding-source required forms
- Use de-escalation techniques and manage crisis situations; notify supervisors for incidents
- Administer drug/alcohol screens when clinically appropriate
- Arrange transportation for appointments when needed
- Support medication monitoring and hourly checks as required
- Participate in trainings, case consultations, staff meetings, and required supervisory sessions
- Maintain and develop working relationships with community social service agencies
Administrative Responsibilities
- Run shifts in the absence of a supervisor and handle on-call duties when scheduled
- Screen and triage incoming calls and route information appropriately
- Maintain client records per agency guidelines and timelines
- Attend required trainings and supervisory meetings
- Complete and submit timesheets and required financial/administrative reports accurately and on time
Minimum Qualifications
- Associates degree + 2 years’ relevant experience required; Bachelor’s degree + 1–2 years’ experience preferred
- Experience working with people in recovery from substance use and/or mental health conditions
- Strong verbal and written communication skills; sound judgment
- Ability to organize and prioritize responsibilities with minimal supervision
- Willingness to obtain and maintain First Aid, CPR, Defensive Driving, and Fire Safety certifications as required
- Ability to provide goal-oriented services and work effectively as a team member
- Bilingual (Spanish/English) strongly preferred — bilingual candidates encouraged to apply
- Must be legally authorized to work in the United States
Compensation & Benefits
- Hourly rate: $24.50 per hour (estimated annual compensation ~$58,450)
