Position Overview

Confidential Client is seeking a full-time Case Manager to work on-site and in the community supporting a supportive-housing program in Midtown Manhattan (Times Square area). This role serves a primarily low-income and formerly homeless population, many of whom live with serious mental illness, HIV/AIDS, or histories of substance use. The Case Manager provides outreach, crisis intervention, benefits coordination, and practical supports that help clients maintain housing stability and increase independence.

  • Location: New York, NY (On-site & field-based)
  • Employment type: Full-time
  • Schedule: Monday–Friday, 9:00 AM–5:00 PM
  • Salary: $44,240.56 – $59,930.96 per year

What You’ll Do

  • Provide person-centered case management for a dedicated caseload, building trusting relationships and developing individualized service plans (ILPs).
  • Conduct regular outreach and community-based visits, including appointment escorts and support for medication self-administration per agency protocols.
  • Assist clients with benefits enrollment and maintenance (SSI/SSDI, Medicaid, SNAP, HASA) and support financial skill-building, including rep payee coordination.
  • Support housing retention: move-in orientation, lease recertifications, inspection preparation, landlord communications, and daily living skills (hygiene, cleaning, hoarding mitigation).
  • Deliver trauma-informed crisis intervention and coordinate emergency services when needed.
  • Facilitate support and recreational groups and complete timely, accurate EHR progress notes to meet contact and documentation targets.
  • Collaborate with interdisciplinary staff and community partners to connect clients with clinical, medical, and social resources.

Qualifications

  • High school diploma or GED required; Bachelor’s degree in Social Work, Psychology, or related field preferred.
  • Entry-level candidates are encouraged to apply; prior experience with supportive housing, behavioral health, homeless services, or vulnerable populations is a plus.
  • Demonstrated communication and interpersonal skills; ability to de-escalate crises and work effectively in clients' homes and community settings.
  • Comfortable managing confidential information and maintaining professional boundaries.
  • Bilingual Spanish preferred but not required.
  • Competency with basic computer tools (Microsoft Office, EHR systems) and caseload documentation.
