What You’ll Do
This Case Manager role supports individuals and families experiencing homelessness, mental illness, and complex needs. You will provide person-centered case management on-site and in the field across New York City, focusing on housing stability, benefits access, crisis intervention, medication support, and life-skills coaching.
- Build rapport and maintain a dedicated caseload; develop and update individualized service plans (ILPs).
- Conduct outreach and accompany clients to community appointments (medical, legal, benefits).
- Support medication self-administration and coordinate with clinical providers as needed.
- Assist clients with public benefit applications and recertifications (SSI/SSDI, Medicaid, SNAP).
- Manage representative payee funds and help clients track financial goals.
- Support move-in orientation, daily living skills (hygiene, cleaning), hoarding mitigation, and pest/bed-bug remediation processes.
- Provide crisis intervention, coordinate with emergency services and discharge planners.
- Facilitate groups and recreational activities; document timely EHR progress notes to meet contact-hour targets.
Qualifications & Requirements
- Education: High school diploma or GED required. Bachelor's in Social Work, Psychology, or related field preferred.
- Experience: Entry-level — no formal experience required. Prior exposure to human services or supportive housing preferred.
- Competencies: strong communication and interpersonal skills, trauma-informed de-escalation, ability to work independently in the field, and comfort with home visits.
- Language: Bilingual Spanish preferred but not required.

Compensation & Schedule
- Compensation: $44,240.56 - $59,930.96 per year
- Employment type: Full-Time, Direct Hire
- Schedule: Standard business hours (9:00 AM – 5:00 PM); shift: First
