What You’ll Do
Under direct supervision, the Case Manager II provides primary support and case management for adults with mental illness and co-occurring disorders. You will help clients live as independently as possible in the community by coordinating services, providing outreach, and connecting people to resources.
- Provide strengths-based case management and ongoing support
- Conduct community outreach and coordinate referrals to housing, medical, and behavioral health services
- Accompany or arrange transportation for clients to appointments, groups, and meetings
- Maintain accurate client records, complete documentation and progress notes
- Participate in team meetings, supervision, and trainings
- Support transitions from inpatient or congregate settings to outpatient/community care
- Collaborate with hospitals, mobile crisis teams, and community partners to reduce risk and avoid unnecessary hospitalization
Shifts & Schedule
Full time. Example shift patterns include:
- Tuesday–Saturday, 8:00 AM – 4:30 PM
- Sunday–Thursday, 8:00 AM – 4:30 PM
Compensation
Expected starting wage range: $18.69 – $22.52 per hour. Full wage range for this role can go up to $26.36 per hour. Pay differentials may apply based on geographic location and experience.
Estimated annual (for context): approximately $51,546 per year (varies with hours and location).
What You Bring to the Table (Minimum Qualifications)
- High school diploma/GED plus two (2) years of direct services experience in mental health or community services; OR
- Associate degree plus one (1) year of relevant experience; OR
- Bachelor’s degree in Social Services or related field
- Valid driver’s license and personal vehicle insurance with your name listed as a driver
- Willingness to use personal and/or company vehicles for client transport and job-related travel
- Demonstrated ability to work effectively with diverse populations and community partners
Nice-to-Have / Preferred
- Experience in community-based mental health, diversion, or transition services
- Experience working with clients during discharge planning, crisis diversion, or housing transitions
- Coursework or certifications related to case management, trauma-informed care, or peer support
