What You’ll Do

Under direct supervision, you will provide community-based case management and primary support to adults and youth with mental health and co-occurring disorders. You will help clients access services, develop independent living skills, coordinate care with community partners, and support recovery goals through compassionate, trauma-informed practice.


Responsibilities

  • Conduct outreach, engagement, and strengths-based case planning with individuals and families
  • Coordinate referrals and appointments with clinical teams, social services, housing, and community agencies
  • Transport clients as needed (personal or company vehicle) and document mileage per policy
  • Maintain timely, accurate case records and progress notes in the client information system
  • Monitor client safety and provide crisis support or connect to crisis teams when appropriate
  • Participate in supervision, team meetings, and trainings
  • Support program-level goals at the Kern Youth Family Resource Center and similar community programs

Qualifications (Minimum)

  • High school diploma or GED plus two (2) years of direct services in mental health or community services; OR
  • Associate degree plus one (1) year of relevant experience; OR
  • Bachelor’s degree in Social Services or related field
  • Valid, current driver’s license and personal vehicle insurance with your name listed as a driver
  • Willingness to use a personal vehicle or company vehicle to transport clients when required
  • Demonstrated ability to work respectfully with diverse populations and to follow supervision and program policies

Compensation & Schedule

  • Expected starting wage: $23.00 - $23.71 per hour. Confidential client applies geographic pay differentials; final starting pay is based on location and experience.
  • Shift example: AM shift 8:00 AM - 4:30 PM, Days: Sunday – Thursday. Both full-time and part-time schedules may be available depending on program needs.
  • Note: The posted pay range is the employer-provided estimate for this role in California.
