What You Will Do to Change Lives

Under direct supervision, the Case Manager II provides primary support and case management for individuals with mental illness and co-occurring disorders. Your work will help clients live as independently as possible in their communities.

  • Conduct outreach, engagement, and ongoing case management with program participants
  • Develop and maintain person-centered service plans focused on housing stability and community integration
  • Coordinate referrals to behavioral health, medical, housing, and social services
  • Transport clients to appointments and community activities when necessary (using personal vehicle)
  • Maintain accurate client records and progress notes in compliance with program and regulatory requirements
  • Participate in team meetings, supervision, and trainings

Schedule & Compensation

Full-Time | 8:00 AM - 4:30 PM | Monday - Friday

  • Expected starting wage: $23.00 per hour (geographic differentials may apply; starting pay commensurate with experience)

Minimum Qualifications (Must Have)

  • High school diploma/GED plus two (2) years of direct services in mental health or community services; OR
  • Associate degree plus one (1) year of relevant experience; OR
  • Bachelor’s degree in Social Services
  • Valid, current driver’s license and personal vehicle insurance with you listed as a driver
  • Willingness to use your personal vehicle for client transport and work-related travel
