What You’ll Do
Under direct supervision, the Case Manager II (Mental Health 355) provides primary support and case management to adults with serious mental illness and co-occurring disorders. You will help clients live as independently as possible by connecting them to services, coordinating care, and delivering community-based supports.
- Conduct outreach and provide community-based support (approximately 80% field work)
- Develop, implement, and monitor individualized service plans
- Link clients to benefits, housing, medical and behavioral-health services, and community resources
- Provide client transport when needed using a personal vehicle (mileage reimbursed per client policy)
- Maintain timely documentation and case notes in electronic records
- Collaborate with multi-disciplinary teams, external providers, and justice-system partners
- Support engagement, stabilization, and steps toward self-sufficiency
Schedule & Compensation
- Location: Philatelic, CA
- Employment type: Full-time — Days (Shifts: 8:30 AM – 5:00 PM; typical schedule: Sunday–Thursday)
- Hourly pay: $23.00 – $23.71 (starting wage range; final offer commensurate with experience)
- Sign-on bonus: $3,000 (see eligibility rules below)

Minimum Qualifications (Must Have)
- High school diploma or GED plus two (2) years of direct services in mental health or community services; OR
- Associate degree plus one (1) year of experience; OR
- Bachelor’s degree in Social Services (or related field)
- Valid, current driver’s license and personal vehicle insurance listing you as a driver
- Willingness to use a personal vehicle to transport clients to appointments and groups (mileage reimbursement per policy)
