What You’ll Do
This is an entry-level Case Manager role supporting members, families, and service providers in a mission-driven, regulated care environment. You will provide administrative and case coordination support while receiving on-the-job training and professional development.
- Support members, families, and service providers through professional communication by phone, email, virtual meetings, and occasional in-person interactions.
- Assist with coordinating care plans, referrals, follow-up appointments, and service updates.
- Maintain accurate case records and documentation within electronic case management systems.
- Perform data entry, record updates, and administrative tasks while ensuring compliance with organizational and regulatory standards.
- Track assigned activities, assist with reporting, and support quality assurance initiatives.
- Collaborate with team members to ensure continuity of services and timely completion of case management activities.
- Participate in onboarding, training, team meetings, and ongoing professional development.
- Monitor assigned tasks, escalate concerns when appropriate, and contribute to a positive member experience.
- Utilize technology and case management software to maintain accurate documentation and workflow efficiency.
- Perform additional duties as needed to support the overall success of the team.
Qualifications
Required (one of the following):
- Bachelor’s degree
- Five (5) years of relevant experience supporting individuals in Long-Term Services and Supports (LTSS), Developmental Disabilities, or a related field
- An equivalent combination of education and applicable experience
Additional skills and attributes:
- Strong customer service and interpersonal communication skills
- Excellent organizational skills with strong attention to detail
- Ability to prioritize multiple tasks while meeting deadlines
- Comfortable using Microsoft 365 and electronic documentation systems
- Ability to work both independently and collaboratively in a hybrid work environment
- Willingness to learn and grow in case management
Additional Details
- Schedule: Hybrid (remote flexibility offered after training; occasional office and community responsibilities)
- Employment type: 6+ month contract with potential for conversion to hire
- Pay: $18 - $20 per hour
- Benefits: Eligible for medical, dental, vision, and 401(k)
